Rates & Insurance

Optimal Health & Performance:

• Consultation fee: based on time (CPT code: 99204/5); approximately $200/hr, minimum of 15 mins (only $100/hr for paraprofessional research); OR…
• Tiered Membership Fee is as follows:
  • Premium program: Our Metabolic Medicine & Hormone Deficiency program; includes a health risk appraisal (HRA), for early detection and early intervention of those risk factors for Metabolic Syndrome; as well as BHRT (“Bio-identical” Hormone Replacement Therapy); this requires establishing a doctor-patient relationship;
  • $97/mo for a single membership and $147/mo for a couple;
  • Maintenance program: after your first year; advice from a Preventive Medicine Consultant as a patient advocate; includes: email correspondence, phone communication, monthly newsletter and telemedical access, as well as continued “fine-tuned” HRT. 
  • $47/mo w/ annual subscription; 

Insurance

Depending on your current Insurance plan, Medicare health plan or Employee benefit plan, it is possible for services to be covered in full or in part. Please contact your provider to verify how your plan compensates you for “Out of Network” Preventive Medicine services, Metabolic Medicine services and Wellness services.

I’d recommend asking these questions to your insurance provider to help determine your benefits:

• Does my health insurance plan include Metabolic Medicine and Wellness benefits?
• Do I have a deductible? If so, what is it and have I met it yet?
• Does my plan limit how many visits per calendar year I can have? If so, what is the limit?
• Do I need written approval for “out of network” services to be covered?

Payment

I accept cash, and all major credit cards as forms of payment. I also accept payment directly on my EHR via credit card only.

Cancellation Policy

If you are unable to attend a scheduled visit, please make sure you cancel at least 24 hours beforehand. Otherwise, you may be charged for a “no show” rate of the visit.
